Medical Treatment Release

The staff at The Animal House Groom & Board will monitor general health conditions whenever your pet receives boarding, daycare, or grooming services. For your convenience, one of our veterinarians from The Animal House Pet Clinic will be available to examine and treat your pet if a problem is noted.

In case of serious injury or illness, The Pet Clinic will immediately provide assessment and a treatment recommendation, and will make every attempt to contact you prior to providing veterinary care.

In an emergency situation, when client contact is attempted but not made, The Pet Clinic will move forward with emergency treatment to the best of its discretion at the owner’s expense.

This release will remain in effect until the pet owner chooses to revoke it.

POLICIES

Safety Protocols
Nothing is more important to us than your pet’s well being during their time with us. We are committed to the highest standards of safety and health.

I understand that:

  • All pets that visit shall have yearly annual exams, up to date vaccinations, and flea protection. Veterinary records must be provided as proof and will be kept on file.
  • Owners must certify that their pets are free of any condition or contagions that could harm another pet.
  • If upon arrival, parasites are detected, topical and systemic parasite prevention will be administered at the owner’s expense, otherwise the pet will not be admitted to daycare or boarding services.
  • Pets should be on a leash no longer than 6 feet when entering and exiting the facility. Retractable leashes are not allowed.
  • Pets must wear a quick release collar complete with nametag.
  • Dogs may experience diarrhea from a change in diet. The best way to prevent this is to bring food from home. Food should be stored in a labeled “Tupperware” type container, and pre-measured in “Ziploc” type plastic bags.
  • In the absence of a diet provided from home, boarding dogs will be fed a premium Low Residue diet that is easy to digest for a nominal daily fee. Pets who are boarding for long periods (3 weeks or more) should consider transitioning to this diet prior to their visit.
  • Cat owners must supply food from home and specify a feeding schedule.
  • Behavior Assessments will be provided to all first time participants in daycare or group play. If a pet does not meet temperament testing criteria they will not be eligible. If your pet is absent from daycare or group play for a period longer than 3 months, a Behavior Assessment may need to be repeated at our discretion.
  • Puppies may start daycare at four months of age, and as long as they are up to date on all required vaccinations. They may start boarding at any age. All puppies over six months of age must be spayed or neutered.

Payment Policies

  • Payment forms accepted: cash, checks (in-state only), MasterCard, Visa, Discover, and Care Credit.
  • All fees are due and payable at the end of each stay. Prepayment can also be arranged.
  • Prepayment is required on boarding stays longer than 10 days.
  • The Animal House Groom & Board reserves the right to apply cancellation/late fees (cost of one day of service) to owners who consistently cancel at the last minute or arrive late to pick up their pets. During holidays deposits may be required.
  • In the event of an outstanding balance past 30 days due, a 1.5% APR will be applied.

Liability Release

I hereby release The Animal House Groom & Board from all liability of injury to my pet or myself.

I acknowledge and understand that:

  • Pets by nature are unpredictable, and without warning may cause injury to humans or other animals.
  • Behavior assessments and health checks are rigorously conducted but cannot be expected to be foolproof.
  • Any condition detected that is deemed contagious and potentially harmful may result in my pet being moved from The Groom & Board to The Animal House Pet Clinic and isolated so as not expose the healthy pets.
  • As the pet’s owner it is my responsibility to keep my pet properly restrained and under control at all times and to prevent any aggressive action toward any other pets and/or people, including Animal House staff.
  • Dogs not regularly exposed to the intensity of playtime activity may experience sore paws and muscles, and fatigue.
  • Excessive intake of water after daycare or boarding may result in vomiting and/or diarrhea.
  • Toenails should be trimmed regularly to avoid injury at owner’s expense.
  • Dogs, especially those with longer coats, who use boarding and/or daycare may require more frequent grooming services.
  • Pets can get wet and dirty when playing outside.
  • Any inappropriate or aggressive behavior from my pet may result in dismissal from group play in boarding, daycare or training.
  • If accepted into daycare, my dog will have contact with new dogs entering the playgroup and may be used in the behavior assessment process.
  • My pet may be exposed to communicable diseases such as conjunctivitis, canine influenza and bordetella among other illnesses.
  • The Groom & Board has relied on honest representation of your pet’s behavior and health history.
  • In case of serious injury, The Pet Clinic will immediately provide assessment and a treatment recommendation, and will make every attempt to contact you prior to providing veterinary care.
